Helen Porter Rehabilitation & Nursing, part of the University of Vermont Health Network and Porter Medical Center, is seeking an experienced and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Post Acute Care Unit on the night shift.

Our post-acute unit provides skilled nursing and rehabilitation services to patients recovering from surgery, illness, injury, and hospitalization, helping them regain strength and independence before returning home. As a member of our nursing team, you'll play a vital role in delivering high-quality clinical care while supporting patients through an important stage of their recovery journey.

Located in beautiful Middlebury, Vermont, Helen Porter Rehabilitation & Nursing is known for providing exceptional skilled nursing, rehabilitation, and long-term care services in a welcoming, resident-centered environment.

Schedule

  • Full-Time
  • 36 hours per week
  • Night Shift: 6:45 PM - 7:15 AM
  • Rotating weekends
  • Three 12-hour shifts per week

What You'll Do

As a Night Shift RN or LPN on our Post Acute Care Unit, you will:

  • Provide skilled nursing care to post-acute and short-term rehabilitation patients
  • Assess, monitor, and document patient conditions and response to treatments
  • Administer medications, IV therapies, and treatments as ordered
  • Collaborate with physicians, nurse practitioners, therapists, and interdisciplinary team members
  • Support patient recovery goals and discharge planning
  • Respond to changes in patient condition and communicate effectively with providers
  • Promote patient safety, comfort, and quality outcomes
  • Maintain accurate electronic medical records and nursing documentation

Qualifications

Registered Nurse (RN)

  • Current Vermont RN license or eligibility for Vermont licensure
  • Associate's or Bachelor's degree in Nursing

Licensed Practical Nurse (LPN)

  • Current Vermont LPN license or eligibility for Vermont licensure
