Job description

The Registered Nurse, RN is responsible for the delivery of patient care including triaging patient needs over the phone, making visits to patients with changing conditions, and performing admissions as necessary. This role offers the chance to work within a compassionate and close-knit hospice team that supports one another and prioritizes high-quality patient care. Casual RNs are welcomed into the team culture and are appreciated for stepping in to support both patients and staff when needed. One year of nursing experience preferred but not required.

Opportunity Details:
  • Schedule: PRN/On-Call - Flexible Schedule - Pick up shifts as you're able to work them
  • Minimum Requirement - 2 Full Shifts per month - Days 8am - 4:30pm
  • Pay: The targeted pay range for this job is $38.50 - $43.50 per hour.

Essential Job Responsibilities:
1. Receives and responds to phone calls during assigned shifts as well as making visits to patients with changing conditions and completing weekend admissions.
2. Provide compassionate, end-of-life care focused on dignity and comfort
3. Educate and support patients & families on Hospice philosophy, pain/symptom management, and end-of-life expectations.
4. Deliver care in homes, assisted living, and skilled nursing settings.
5.. Enjoy autonomy in practice with strong interdisciplinary team support.
6. Administers medications and treatments as prescribed by the physician in the physician's plan of care. Records pain and/or symptom management changes/outcomes as appropriate.
7. Initiates the plan of care and makes necessary revisions as patient status and needs change. Communicates with the physician regarding patient needs and reports changes in the patient's condition; seeks physicians' orders as required.
8. Provides health care instructions to the patient as appropriate per assessment and plan.
Driving Requirements: Local Travel and Driver's License Required
Registered Nurse HSP Minimum Required Qualifications:
• Graduate of an accredited Registered Nursing program
• 1 year of nursing experience in post-acute care setting
• Must be licensed as a Registered Nurse within the state in which working and must maintain licensure.
• Valid Drivers License, proof of insurance and satisfactory motor vehicle record
• Ability to communicate effectively both verbal and written
• Proficient with Microsoft Office Suite
• Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Home Care, Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and SDS, ADA HUD policy and procedures, Fair Housing laws and regulations.
Preferred Qualifications:
• Bachelor of Science degree in Nursing
• One year nursing experience in Hospice, palliative care or serious illness
