Job Description
Reporting to the Pharmacy Director, RN Home Infusion is responsible for providing direct and indirect care to SBL Home Infusion patients. Performs patient or office related duties including but not limited to initiating and maintaining intravenous lines and tubing, administering medication and fluid therapy, educating patients and families online maintenance and treatment, review patient records, validate medication lists and histories, ensure proper documentation, and monitoring for reactions.

Responsibilities
Actively participates in hospital-wide performance improvement activities; ensures participation by employees. Demonstrates development of effective performance improvement programs and quality control programs., Acts a liaison between patient, family, staff and physician., Assists customers with inquiries, questions and concerns and works to resolve problems in a timely manner., Assists with the continuity of care and discharge planning, Assures quality patient care and ensures that sufficient and properly trained persons carry out the functions of the department., Coaches and counsels employees, as needed, to ensure optimal patient care and maximum efficiency in department fashion. Evaluates staff performance, disciplinary actions, and separation of service., Communicates changes in policy, procedures and other appropriate communication to staff and provides feedback from staff. Participates in policy/procedure review, update, and implementation., Confers regularly with the Prairie Pharmacist and Pharmacy Director regarding all purchasing issues including product availability, recalls, contract changes, new products, etc., Coordinates all staff, scheduling and monitoring of personnel to ensure appropriate staffing on a daily basis., Coordinates and oversees operation strategy to include daily administration and issue resolution of Home Infusion Services., Coordinates supply and pharmacy requests from Home Health Agencies for Home Infusion patients. Obtains supply orders from patients and/or nursing staff and assembles supplies., Delegates responsibilities within scope of personnel abilities., Demonstrates personal and professional growth by acquiring new skills and knowledge required to adapt to regulatory changes (HCPC coding; Medicare allowable charges, etc.), Ensures the provision of safe, financially sound and efficient patient care., Inputs and maintains patient information (i.e., patient demographics, insurance, patient clinical, etc.) in the MBI system and patient file, in accordance with Joint Commission standards., Insures appropriate information is received from referring organization (i.e., 485 Form, physician orders, Hospice Plan of Care Form). Organizes appropriate consent forms and clinical information for infusion patients., Is fiscally responsible for the cost effectiveness of care provided and for administration of the departmental budget., Is responsible for providing leadership and promoting effective relationships between departmental staff, physicians and other departments., Maintains required records and reports and submits in a timely fashion (budgetary, training, performance improvement, productivity, etc.)., Plans and assists in patient teaching related to Home Infusion., Promotes an environment in which the Home Infusion Team can work cooperatively toward objectives. Maintains communication and promotes cooperation with other departments., Responsible for daily/monthly accounting functions in the MBI System. Responsible for assuring that the MBI System is running properly and producing accurate and timely data and documents. , Seeks assistance form appropriate resource person as necessary in solving clinical and management problems., Sets specific standards and objectives for the Home Infusion Department
