Overview

Description

Scope: The Staff RN PRN performs diverse duties in a personal care home setting. This Nurse works as a professional practitioner employed at Skyland Trail in Residential Services to assume responsibility and accountability for the application of the Nursing process and the delivery of client care. Is able to carry out clinical recommendations for care consistent with policies and procedures of Skyland Trail.

Duties:

  • Utilizes the nursing process to assess, implement and evaluate client care.
  • Implements client treatment plans and discharge planning for residential clients.
  • Serves as the liaison with community Psychiatrist and Doctors. Is available to take medication orders from attending Psychiatrist.
  • Administers medications and monitors self-administration of medications at residential sites and the Health and Education Center
  • Performs medical and health screenings, as indicated. Collects specimens, vital signs and weighs clients. May give injections as needed.
  • Assess signs and symptoms indicating physiological and/or psychosocial changes in the clients condition
  • Follows up on emergency, clinic and hospital visits
  • Documents client responses to nursing and other interventions
  • Records and reports observed symptoms, reactions, treatments, and changes in the clients condition.
  • Orient clients and families to the facility. Provides for the safety of clients.
  • Monitors activities of visitors.
  • Performs room searches, as necessary.
  • Instructs clients in activities of daily living, interpersonal and socialization skills
  • Assist in crisis situations in coordination with the crisis communication plan.
  • Intervenes as necessary to decrease panic of clients.
  • Co-Facilitates an educational group per week for clients
  • Participates in change of shift reports and ensures that medication management policies are followed.
  • Facilitates/Co-Facilitates community meetings.
  • Serves as the alternate residential representative to the Safety and/or QI Committee if needed.
  • Participates in Infection Control and Medication Management Policy and Procedures reviews.
  • Performs other duties as assigned that are within the scope of this employee's qualifications.
  • Maintains and stocks inventory of office and/or clinical supplies as appropriate to the position.
  • Supports professional practice by observing the following: Uses knowledge and practices in accordance with 1)the Georgia Practical Nurses Practice Act 2)Standards of Nursing Practice and Patient Care 3) Standards of Patient Care 4) JCAHO, ORS, state and local laws 5)Skyland Trail Policy and Procedure

Reports to: Residential Nursing Supervisor

Competencies: This position should have sufficient preparedness in the following areas:

  • Current License RN for Georgia
  • Current CPR and First Aid certifications
  • Best Nursing Practices
  • Crisis Management Skills
  • Knowledge of emergency and safety procedures for fire, accidents, disaster and unplanned incidents

Please note this position is PRN and is not eligible for benefits.
