This remote role requires a compassionate professional eager to provide high-quality outpatient mental health services to diverse populations, including adults and youth. Key Responsibilities
  • Conduct thorough and ongoing assessments to evaluate the mental health needs and progress of clients.
  • Facilitate effective communication with supervisors, direct care staff, family members, and other stakeholders regarding client care and progress.
  • Provide a schedule of appointments, home visits, and observations for supervisors to monitor treatment progress.
  • Coordinate and execute home visits that comply with in-home observation requirements, including chart reviews and interviews.
  • Ensure that treatment plans are implemented effectively and that any necessary adjustments are made.
  • Take part in Interdisciplinary Team Meetings and contribute valuable insights about patient care.
  • Complete monthly and quarterly progress notes that document client advancements accurately.
  • Organize and lead training sessions to certify staff in the implementation of program plans.
  • Conduct follow-up interviews and observations to provide sustained support for clients’ evolving needs.
  • Maintain accurate and easily accessible records to support client care and collaboration across the healthcare team.
  • Act as a liaison with the mental health community to promote development goals and facilitate support networking.
  • Assist in developing training materials aimed at enhancing program effectiveness for individuals.
Patient Population / Client Focus

Your role will engage with a wide array of client demographics, accommodating individuals across all age groups facing mental health challenges. This includes but is not limited to clients dealing with anxiety, depression, trauma, substance misuse, and relational issues. Qualifications
  • A Master’s or Doctoral degree in Counseling, Psychology, Social Work, or a related mental health field is required.
  • Active and unrestricted licensure as a Professional Counselor in the state of Oregon is essential.
