Job description

The Outreach Coordinator plays an important and unique role in supporting Population Health initiatives through patient outreach and engagement. This role is responsible for supporting patient recruitment, retention, and engagement strategies that contribute to improved population health outcomes. The Outreach Coordinator will conduct outreach activities that connect current and prospective patients with NEMS services, promote preventive care, and increase access to culturally and linguistically appropriate healthcare resources.

The candidate must demonstrate strong communication and relationship-building skills to work effectively with patients and families, healthcare providers and care teams, and community-based organizations. These skills will support improved care coordination, help identify and address cultural and healthcare system barriers, promote timely access to care, and empower patients to engage in preventive services and population health programs. 

ESSENTIAL JOB FUNCTIONS:

  • Conduct outreach to newly assigned patients to establish primary care services and connect them with appropriate resources and services at NEMS sites.
  • Conduct targeted outreach to patients to support population health initiatives, including annual wellness visits, preventive screenings and services, chronic disease management, and care gap closure.
  • Conduct outreach to diverse patient populations to promote engagement in NEMS services, programs, and population health initiatives, while supporting NEMS marketing and community engagement strategies.
  • Identify and understand the needs, interests, and barriers affecting current and prospective patient populations to increase awareness and utilization of services offered by NEMS.
  • Assist with data collection and tracking of outreach activities, and report relevant statistical and operational information to management and leadership.
  • Track outreach outcomes related to quality measures and population health initiatives to support reporting and performance improvement efforts.
  • Participate in Performance Improvement and Continuous Quality Improvement activities and attend trainings and meetings as needed.
  • Ensures compliance with all applicable local, state, and federal regulations, as well as organizational policies and procedures.
  • Performs other job duties as required by manager/supervisor
  • High school diploma with minimum of 2 years of relevant work experience. Associate or Bachelor’s degree preferred.
  • Experience with medical appointment scheduling required. Experience with Epic EHR scheduling is strongly preferred.
  • Prior experience in a managed care setting or IPA provider organization a plus.
  • Prior experience with customer service; can build rapport with patients via phone.
  • Proficient with Microsoft Office and PC based software programs or automated database management systems preferred.
  • Ability to follow through with delegated tasks and self-manage in a detail-oriented environment.
  • Good organization and prioritization skills to manage multiple projects and timelines.
  • Strong interpersonal skills to collaborate effectively with a broad range of internal and external stakeholders.
  • Excellent communication (spoken and written) and time management skills.

LANGUAGE:

  • Must be able to fluently speak, read and write English.
  • Fluent in Chinese (Cantonese and/or Mandarin) required.
  • Fluency in other languages is an asset. 

STATUS:

  • This is an FLSA Non-exempt position.
  • This is not an OSHA high-risk position.
  • This is a Full Time position.
