Job description

My Neighborhood Connections
Hannan Center is an activity hub for older adults (60+) to learn, connect and create. Hannan Center offers creative aging programs, events and workshops as well as a full-service social work department to connect older adults to the support they may need to remain healthy and independent. In 2019, Hannan Center launched My Neighborhood Connections, a program designed to help older adults age in place in Southeast Michigan neighborhoods. As defined by the CDC, aging in place refers to the ability to live in one's home and community safely, independently, and comfortably, regardless of age, income or ability level. My Neighborhood Connections AmeriCorps members support this mission by: promoting a culture of health in Southeast Michigan neighborhoods; partnering with residents to identify community assets and challenges to aging in place; developing sustainable neighborhood volunteer networks; connecting residents with community services which promote healthy aging; facilitating arts and wellness workshops and events; conducting senior health assessments, and carrying out other activities that support healthy aging. Preference will be given to candidates who reside in the MNC community areas which include areas of Macomb, Oakland, and Wayne Counties. Applications are being accepted through the AmeriCorps web site only.
Member Duties : All AmeriCorps. Members are expected to perform service hours Monday - Friday, between 9am-5pm, at the Hannan Center, or in the field at a specified partner site. Occasionally some evenings and Saturdays may be required for mandatory events. Connect residents with affordable services and support to successfully age in place. Identify and develop community partnerships with local universities, and other community institutions. Conduct screenings and intake surveys to connect residents with appropriate programs or wrap-around services. Conduct home assessment surveys to identify environmental determinants to healthy aging. Connect residents with Hannan Center's Home Accessibility Modification Program. Establish My Neighborhood Connections affiliates in southeast Michigan neighborhoods. Track volunteer efforts and peer support using Hannan Center databases Design curriculum and facilitate arts and wellness instruction for neighborhood residents.
