How much do needle exchange jobs pay per hour?

As of Aug 19, 2026, the average hourly pay for needle exchange in the United States is $17.85, according to ZipRecruiter salary data. Most workers in this role earn between $15.38 and $18.27 per hour, depending on experience, location, and employer.

What is a needle exchange?

A Needle Exchange job involves working at harm reduction programs that provide clean syringes to individuals who use injectable drugs. The goal is to reduce the spread of bloodborne infections like HIV and hepatitis while offering resources for healthcare, treatment, and social services. Workers may also educate participants on safe injection practices and offer referrals to addiction treatment programs. The role requires empathy, nonjudgmental communication, and knowledge of public health practices.

What skills and qualifications are needed for a needle exchange position?

To thrive in a Needle Exchange role, you need knowledge of harm reduction principles, public health education, and safe handling of biohazard materials, often paired with training in social work or community health. Familiarity with data tracking systems, sterilization protocols, and potentially certifications such as Bloodborne Pathogens training are valuable. Strong interpersonal skills, cultural sensitivity, and nonjudgmental communication are especially important in building trust with diverse and vulnerable populations. These skills are crucial for providing effective services, ensuring safety, and supporting the overall health and well-being of program participants.

What are typical challenges faced by staff working in needle exchange programs?

Staff in Needle Exchange programs often encounter challenges such as managing high client volumes while ensuring safety protocols are maintained, and addressing the complex needs of individuals who may be experiencing addiction, homelessness, or mental health issues. Building trust and rapport can take time, as clients may be wary of authority or have past experiences of stigma. Team members work closely together and with community partners, offering referrals to healthcare, housing, or counseling as needed. This role requires adaptability and compassion, as every day can present new and unpredictable situations, but it is also highly rewarding for those passionate about harm reduction and public health.

Are there still needle exchange programs?

Yes, needle exchange programs are still active in many regions as a public health measure to reduce the spread of infectious diseases among people who use injectable drugs. These programs often employ trained staff and follow local regulations to distribute sterile needles and collect used ones. Job roles in these programs may include outreach, education, and harm reduction services.
