Job Summary
We are seeking a Remote Patient Monitoring (RPM)/Chronic Care Management (CCM) Care Manager who will be responsible for overseeing our RPM and CCM program. In this fully remote, flexible, role, you will work collaboratively with patients to determine their medical needs, develop the best course of action, and oversee their treatment plans, ensuring each client gets high-quality, individualized care. The ideal candidate is compassionate, patient, and knowledgeable about healthcare practices.ย

Responsibilitiesย 
  • Collaborate with physicians, patients, families, and healthcare staff
  • Coordinate a variety of healthcare programs
  • Review daily measures
  • Oversee a monthly patient roster, ensuring comprehensive care for each individual
  • Aiming for a patient engagement rate of 90% or higher
  • Develop individualized care plans
  • Educate patients on their healthcare options
  • Provide one-on-one coaching, create goals, and support to patients managing chronic conditions, including but not limited to type 2 diabetes, hypertension, and cardiovascular disease.
  • Recruit and train staff
Qualifications
  • Previous experience as a Care Coordinator in RPM/CCM or in a similar position is preferred
  • Comprehensive knowledge of Hypertension, Diabetes, Hyperlipidemia, COPD, Obesity, Arthritis, Chronic Kidney Disease, Asthma
  • Active Certification as a Medical Assistant from an accredited association. (AAMA, AAH, AMT, NHA, NAHP or NCCT)ย 
  • Active compact multi-state LPN/LVN License.
  • Active health coaching certification (NBHWC, NSHC, ACE Advanced) preferred
  • Fluency in second language is a plus
  • Strong problem-solving and organizational skills
  • Ability to manage multiple projects or tasks and prioritize appropriately
  • Ability to work in fast-paced situations and make sound decisions quickly
  • Excellent interpersonal skills and high level of compassion
  • Strong verbal and written communication skills
  • Comfortable learning and using EHR platforms
