Home Health Physical Therapist position with CenterWell Home Health Adult and geriatric patient population Responsible for performing comprehensive patient evaluations and developing individualized Plans of Care (POCs) Provide treatment for neurological, neuromuscular, and orthopedic conditions in the home setting Complete all required documentation including evaluations, progress notes, physician orders, and ongoing clinical records Collaborate closely with physicians, nurses, caregivers, and the interdisciplinary care team Provide patient and family education regarding treatment plans, home exercise programs, safety, and equipment use Assess and recommend appropriate durable medical equipment including walkers, wheelchairs, canes, braces, prosthetics, and other assistive devices Instruct patients and caregivers on safe transfers, mobility techniques, and functional activities Participate in infection control and patient safety initiatives Ability to travel locally throughout the assigned territory Home Health experience preferred OASIS experience preferred Current Physical Therapist license required Current CPR certification required Valid driver's license, reliable transportation, and proof of insurance required 1-2 years of Physical Therapy experience in an acute care or rehabilitation setting required Strong communication, documentation, and clinical assessment skills required Ability to perform physical demands including lifting, bending, pushing, pulling, prolonged standing, and walking Weekly pay package calculated and confirmed prior to submission Medical, dental, vision, and supplemental benefits available Travel reimbursement available Questions or Interested?
