Shift Details: Monday through Friday 0800-1600, Occasional weekends to meet department needs for staff education needs, no holidays
Job Summary:
The MSW Social Worker/Educator is a practitioner and educator. As a practitioner, the MSW Social Worker/Educator is responsible for coordinating care and services of selected patient populations through the acute care episode and across the continuum to support an effective discharge plan. As an educator, the MSW Social Worker/Educator collaborates with Organizational Learning, participating in the design, implementation and evaluation of orientation, continuing education, and in-service education relevant to Care Management staff, patient population(s), practice setting, regulatory compliance, and patient and family education. The Educator uses tools, theories, skills, and knowledge to maintain Care Management standards of practice and to improve the care management practice of learners. The MSW Social Worker /Educator is available for all staff, based on individual educational needs. The MSW Social Worker/Educator serves as a role model and has a responsibility to foster a positive attitude about the benefits of change and opportunities of life-long learning.
EXPERIENCE DESCRIPTION:
A minimum of two years experience in an acute care setting is preferred. Casework in medical social work is required. Candidate should have experience and knowledge of transitional planning, completing psychosocial and needs assessments, readmission reduction, and discharge planning. The candidate also will have the ability to effectively assess staff members' educational needs, design programs to meet identified needs, and to oversee instruction for staff members. Professional knowledge of social work theory, educational theories, and principles to assist in the process of providing education to a professional staff is required.
EDUCATION DESCRIPTION:
Masters Degree in Social Work is required.
SPECIAL SKILLS DESCRIPTION:
Theoretical knowledge of the social work processes. Interpersonal skills necessary to obtain information from patients, to provide counseling to patients/families, and to interact effectively with patients and families. Analytical skills necessary to assess patient's needs, develop associated discharge planning and develop solutions to problems concerning patients mental and physical well-being. Advanced communication and interpersonal skills with all levels of internal and external customers including but not limited to medical staff, patients and families, clinical personnel, support and technical staff, outside agencies and members of the community. Other skills needed include excellent written/verbal communication skills, critical thinking, creative problem solving, proficient organization and planning. Basic computer skills. Must be self-directed with ability to tolerate frequent interruptions and a demanding workload.
LICENSURE DESCRIPTION:
Requires current state of Wisconsin Clinical Social Worker (LCSW) or Advanced Practice Social Worker (APSW) certification.
