## MSW Discharge PlannerApplyremote type: 1- Onsitelocations: Baptist Medical Center Southtime type: Full timeposted on: Posted Todayjob requisition id: JR106802Baptist Health is the largest healthcare system serving central Alabama, providing comprehensive hospital-based and outpatient services to nearly 60 percent of the residents in Montgomery, Autauga and Elmore counties.To learn more about Baptist Health, visit us at https://www.baptistfirst.org.**Highlights:****Summary**: The Discharge Planner, MSW shall coordinate care of an assigned caseload of patients through collaboration with physicians, nursing staff, and other health professionals. The Discharge Planner will facilitate efficient utilization of clinical resources, optimal clinical and continuing care, as well as financial outcomes. This individual shall be responsible to provide a collaborative process of assessment, plan, facilitate, care coordination, evaluate, and advocacy for options and services to meet an individual’s and family’s comprehensive human service needs through communication and available resources. This position may have additional duties assigned that are within scope of the role.**Education/Experience**:Masters Degree in Social Work required. Minimum 3 years of hospital experience preferred. Knowledge of case management. Experience with skilled nursing facility, home health, hospice, child/adult/elder abuse and neglect, and rehabilitation preferred.**License/Certification**:Active LMSW or LICSW required or obtain within 12 months in position, CCM or ACM Case Management certification preferred**Knowledge, Skills, and Abilities**:* Excellent knowledge of MS Office and familiarity with relevant computer software.* Strong team leadership skills, strong organization, interpersonal, and customer relations skills.* Must be able to meet strict deadlines.* Strong analytical and problem-solving ability, and excellent organizational skills.* Must be able to complete multiple tasks simultaneously.* Ability to develop trust with leaders in the organizations.* Ability to remain calm in difficult situations.* Strong written and oral communication skills, proficient computer skills, proven track record of successful performance.* Ability to understand complex multifactor situations and bring appropriate solutions.* Ability to work both independently and in a team environment.* Knowledge of HIPAA standards and regulations.**Primary Location:**Baptist Medical Center South**Job:**MSW Discharge PlannerDischarge Planner, MSW**Job Type:**Regular-Full time**Shift**:First Shift (United States of America) #J-18808-Ljbffr
