Healthcare Support is searching for a Registered Nurse to join a large organization transforming primary care through longer patient visits with smaller patient volume, coordinated care, and a strong focus on quality care!

Compensation Notes: $87,000 - $90,000+ (based on experience)
Sign On Bonus: $5,000 Sign on Bonus!
Shifts Available: Monday - Friday, 8a-5p
Daily Responsibilities For The RN Case Manager
  • Coordinate end-to-end case management for patients, including care plan development, specialist and diagnostic referral navigation, chronic disease support, patient education, and follow-up to improve health outcomes and ensure a seamless care experience
  • Collaborate closely with an integrated primary care team to provide hands-on clinical support (assessments, injections, phlebotomy, IV fluids, vital signs), while delivering compassionate, patient-centered care through both in-office and virtual interactions
