• Conduct comprehensive clinical assessments for hospice patients, taking into account both psychological and emotional needs.
• Develop individualized treatment plans that address the unique challenges faced by patients as they navigate serious illness.
• Provide evidence-based therapeutic interventions, drawing upon a wealth of methodologies tailored to each patient's situation.
• Maintain meticulous and timely clinical documentation to ensure continuity of care and compliance with regulatory standards.
• Collaborate with a multi-disciplinary treatment team, including nurses, physicians, and chaplains, to提供 holistic patient support.
• Educate patients and their families on coping strategies, available resources, and community support options.
• Stay abreast of current research and best practices in hospice and palliative care to enhance the treatment offered.

The clients you will serve encompass a diverse range of individuals facing terminal illness. Many patients experience complex emotional and mental health challenges as they approach the end of life, and your role will be to provide them and their families with the necessary support and resources to cope with their circumstances. This role demands empathy and an understanding of the psychological impact of serious illness, as well as a commitment to improving patients’ overall quality of life during a critical time.

• Master's degree in Social Work (MSW) from an accredited institution.
• Valid Licensed Clinical Social Worker (LCSW) license in the state of Kansas.
• Strong knowledge of solution-based therapy techniques and best practices in the care of patients in hospice settings.
• Outstanding communication, problem-solving, and interpersonal skills to effectively engage with patients and their families.

• Previous experience working in hospice, palliative care, or end-of-life services is highly preferred.
• Familiarity with trauma-informed care approaches and family dynamics during grief and loss.
• Experience working within a remote healthcare delivery system is a plus.
