Position Summary:
We are seeking a Physical Therapist to provide in-home care for Medicare Part B patients. Help patients regain mobility, manage pain, and live independently-all from the comfort of their homes.
Key Responsibilities:
  • Assess patients in their homes and create personalized treatment plans.
  • Provide skilled therapy to improve mobility, strength, balance, and independence.
  • Educate patients and caregivers on exercises, safety, and fall prevention.
  • Document progress and ensure compliance with Medicare Part B regulations.
  • Collaborate with the home health/ therapy team to coordinate care.
  • Adjust treatments based on patient progress and needs.

Qualifications:
  • Active Physical Therapy (PT) license in the state of practice.
  • Experience in home health or outpatient therapy preferred.
  • Knowledge of Medicare Part B documentation and regulations.
  • Strong assessment, treatment planning, and communication skills.
