Tranquil Minds Wellness is seeking a dedicated Licensed Master Social Worker (LMSW) to join our compassionate team. This position is fully remote, offering a flexible work setting from the comfort of your home while allowing you to maintain essential connections with clients through virtual interactions. You will be working closely with an interdisciplinary team that includes healthcare professionals such as nurses and caregivers, who share a mutual goal of enhancing senior care.

• Conduct comprehensive psychosocial assessments of seniors, considering their emotional, mental, and social status to pinpoint individual needs.
• Collaborate with clients and their families in developing personalized care plans that respect their preferences and goals.
• Provide emotional counseling and support, helping seniors and families manage transitions, losses, and other challenges.
• Advocate for clients by facilitating access to community resources and ensuring their rights are upheld.
• Monitor client well-being through regular virtual check-ins, addressing any concerns or safety issues that arise in their living environment.
• Foster communication among family members to resolve conflicts and improve care experiences for the client.
• Educate clients and families on available resources and support options that promote engagement and enhance quality of life.
• Maintain meticulous records of assessments, interventions, and client progress while complying with confidentiality standards.
• Stay updated with current best practices in social work to ensure the highest standards of ethics and care are followed.

This role focuses on seniors within the hospice setting, who may have complex needs requiring a tailored approach. The ideal social worker will possess a deep understanding of age-related issues, including grief, caregiver stress, and mental health challenges, and will work to ensure these seniors live with dignity, comfort, and companionship.

• A Master’s degree in Social Work from an accredited institution is required.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years’ direct experience in social work, ideally within geriatric care or community service.
• Comprehensive understanding of aging-related issues, social work principles, and community resources available for seniors.
• Exceptional counseling, assessment, and problem-resolution skills are essential.
• Ability to empathize with clients and their families, establishing trust and rapport effectively.
• Strong verbal and written communication skills, facilitating teamwork with diverse groups.
• Familiarity with electronic health record (EHR) systems and relevant software applications is a plus.

Previous involvement in hospice care or with interdisciplinary teams will be beneficial. Experience helping clients transition through chronic illness or end-of-life decisions is highly desirable, including knowledge of community resources and support networks.

The salary range for this position spans from $60,000 to $100,000 annually, commensurate with experience. Additionally, we offer a comprehensive benefits package that includes:
