Job Summary
PACT Midlevel Provider - Nurse Practitioner (NP) or Physician Assistant (PA)
Overview: Aptive Resources, in partnership with Valor Healthcare, is hiring Midlevel Primary Care Providers (Nurse Practitioner or Physician Assistant) to deliver comprehensive primary care to Veterans within the Patient Aligned Care Team (PACT) model at the proposed Yreka, CA Community-Based Outpatient Clinic (CBOC), pending VA contract award.
Staffing Ratio: 1.12 FTE (per this site's staffing plan) - staffed to meet VA panel-size standard of 900 active patients per full-time mid-level provider
Location: On-Site | Yreka VA Community-Based Outpatient Clinic (CBOC), 807 S. Main St., Yreka, CA 96097
Schedule: Monday-Friday, 7:30am-5:30pm (Clinic Operating Hours); 8:00am-4:30pm Patient Schedule Hours
Employment Type: Full-Time Contract
Primary Responsibilities
  • Provide health care commensurate with licensure, clinical privileges, and scope of practice to assigned and unassigned patients presenting for care
  • Ensure the patient's care plan contains clinically indicated medical recommendations and offer all clinically indicated services to PACT-assigned patients
  • Provide leadership to the PACT team, including shared delegation of appropriate care and care processes to other team members
  • Review available clinical and performance data with the team and focus on continuous improvement of critical team processes
  • Ensure patients have same-day access for face-to-face and virtual care visits during regular clinic hours
  • Collaborate with PACT staff to develop personal health plans incorporating care management and coordination appropriate to patient needs
  • Meet performance benchmarks of greater than 85% same-day appointment demand met and greater than 95% of encounters closed in the VA electronic health record (CPRS) within 72 hours

Minimum Qualifications
  • Nurse Practitioner (APRN) option: Master's degree from a program accredited by NLNAC or CCNE; current certification as a Certified Nurse Practitioner (Adult Health or Family Practice) from a nationally recognized certifying body; authorization for prescriptive authority required; minimum 3 years of clinical nursing experience and at least 1 year (3 preferred) of clinical experience as a CNP; outpatient Family or Internal Medicine experience preferred; current, full, active, unrestricted license/registration as a graduate professional nurse in the state where the clinic is located, or meets VA Qualification Standards for Advanced Practice Nurse
  • Physician Assistant (PA) option: Graduate of a program accredited by the Accreditation Review Commission on Education of PA (ARC-PA) or predecessor organization; master's degree in any discipline, or bachelor's degree with 1 year of PA experience; successful completion of the PA National Certifying Exam (PANCE); authorization for prescriptive authority required; current, full, active, unrestricted license/registration in the state where the clinic is located, or meets VA Qualification Standards for Physician Assistant
  • Must hold a current, full, active, and unrestricted license/certification in California, or otherwise meet VA Qualification Standards for the position.
  • Must successfully complete VA credentialing/privileging review and a background investigation (cleared through OPM via the VA Security and Investigations Center) prior to starting work; PIV credentialing required.
  • Must complete required VA mandatory training and maintain current licensure/certification documentation annually.
