Job description

Tile: Youth Transition Specialist
Location: Logansport, IN 46947
Duration: 12+ Months 
Pay rate: $21/hour
Position Overview
The Transition Specialist oversees transition-related practices in a state juvenile correctional facility's Division of Youth Services. This role supports youth reentry into the community through standardized programs, partnerships, and individualized planning.
Key Responsibilities
Facilitate implementation of the "Next Steps" life skills curriculum.
Apply the Transition Coordinator Handbook to standardize transition practices across facilities.
Evaluate five "Transition Consultations" for each student, guided by assigned teachers.
Participate weekly in Administrative Release Committees (ARC) to review and strengthen reentry plans.
Conduct follow-up phone calls to released youth at 1, 3, 6, 9, and 12 months post-release.
Represent the Division of Youth Services at conferences, meetings, and partnership initiatives (e.g., with Department of Workforce Development, HIRE, and University’s HOPE Mentoring).
Assist in developing individualized Education Learning Plans for youth upon facility entry.
Collaorate with state, county, and private agencies to deliver reentry services.
Help students obtain essential documents, including social security cards, state IDs, driver's licenses, and birth certificates.
Support transitions to public schools, employment, apprenticeships, and military service opportunities.
Required Skills and Experience
Strong communication skills (written and verbal) – Required.
Microsoft Word, Outlook, Excel, and PowerPoint – Required.
Customer service representative experience (1 year) – Required.
Data entry experience (1 year) – Required.
Team environment experience (1 year) – Required.
Working with at-risk youth – Highly Desired.
Community or state program experience – Highly Desired.
Youth educational program experience – Highly Desired.
