Job description

BASIC FUNCTION: 

Under the supervision of the Group Home Nurse Manager, ensures that the health and medical needs of the individuals of the assigned group homes are met.  Also ensure that medication administration is completed at Milestone Industries and necessary health information is relayed to group homes.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  •  Reliable and prompt attendance, demonstrating flexibility as necessary to meet the needs of the individuals and/or home.
  • Collects and safely stores medications as individuals arrive at Milestone Industries.
  • Gathers necessary information regarding administration of PRN medications that occurred at individuals group home.
  • Administers regularly scheduled medications to Individuals while attending Milestone Industries.  Also administers necessary PRN medications as prescribed.
  • Distributes back medications to appropriate staff along with any and all documentation regarding health.
  • Completes first aid and or triage assessment for individuals as they are attending Day Training.
  • Reviews general health status, vital signs, medications and treatments given.
  • Reviews injury reports involving group home individuals and makes recommendations for any additional medical interventions needed.
  • Reviews medications on a monthly basis.
  • Reviews medication errors, reports errors to appropriate Group Home Administrator and RN Trainer.
  • Performs annual vision and hearing screenings for individuals in an ICF (Searles and Sun Valley)
  • Coordinates laboratory orders reviews and records results, notifying physicians and RN Trainer of any immediate action needed based upon results.
  • Demonstrates knowledge of Public Health and Public Aid Standards, Department of Human Services and the Nursing Home Care Reform Act providing guidance to and working cooperatively with other staff to ensure continuous compliance with all applicable regulations.
  • Actively participates in in-service training and departmental meetings, promoting positive problem solving and cooperative team efforts.
  • Communicates with pharmacist on an as needed basis in regards to medication orders and MAR
  • Records nursing needs of the individual in Health Status to assure continuity of care.
  • Communicates and trains all professional and non-professional staff concerning the comfort and wellbeing of the individuals as needed regarding health care issues and procedures.  Communicates with and trains all staff in general health and safety.
  • Takes and records phones orders from physician.
  • Monitor individual’s refusal of medication.
  • Functions as a member of the IDT in developing individual treatment plans for the individual group homes.  Prepares assigned semi-annual/annual assessments and reports.
  • Maintains necessary qualifications to remain licensed in the state of Illinois.
  • In compliance with Milestone, Inc. philosophy and policies regarding human rights, immediately intervenes on behalf of the individual(s) served and reports to the appropriate agency (OIG, DCFS, IDPH) and/or supervisor.
  • Ensures that assigned individuals has an annual physical and dental examination and reviews the results of these exams.  Also reviews results of routine laboratory screening and special tests ordered by the individual’s physician.
  • Assists with the admission, discharge and transfer of individuals.
  • Ensures that PRN medication protocol per RULE 116 is documented specifically on MAR
  • Assists RN-Trainer as needed in performing an assessment of the individual’s physical and mental status and medicals history and health care needs plus evaluate the medication orders and medication prescribed.
  • Communicates with physicians and other health professionals as needed.
  • Attend semi-annual, annual, and IDT meetings.

QUALIFICATIONS:

  • Licensed Practical Nurse or Registered Nurse
  • Licensed in the State of Illinois
  • Must be able to physically intervene in a health or behaviorally related crisis.
  • Prefer one year experience with persons with developmental disabilities.
