Home Health Occupational Therapist (OT) | PRN | Caprock Home Health + AccrediMatch | Lubbock, TX

$60 per visit. You pick the visits, you pick the days.


Caprock Home Health (CHHS) is adding Occupational Therapy capacity in Lubbock and is partnering with AccrediMatch to bring on PRN OTs visit by visit. If you are a licensed OT in the Lubbock area and you want to add a few home health visits to what you are already doing, this is built for you.

Already have a full-time OT job somewhere else? That is exactly who this opening is for. Pick up two visits a week, or twenty. Work the visits that fit around your schedule, your commute, and your life. There is no minimum commitment. You can stop and start whenever you need to.

If you want to build this into a larger caseload, that is also on the table. Caprock is a long-standing Lubbock home health agency with steady visit volume across the West Texas market, and AccrediMatch will keep routing Caprock visits to you as long as you keep accepting them.

What you will do on each visit

•       Evaluate patients in their homes for functional status, activities of daily living, fine motor coordination, and home and adaptive equipment needs

•       Develop and update the occupational therapy plan of care in coordination with the patient's physician and the Caprock care team

•       Deliver skilled OT interventions consistent with the plan of care and applicable Medicare Conditions of Participation

•       Complete required assessments and reassessments on the intervals specified for home health OT

•       Document each visit accurately and on time in the agency's EMR (WellSky preferred)

•       Coordinate with the Caprock supervising RN and the rest of the interdisciplinary team as care progresses


Requirements

•       Active Texas Occupational Therapist license

•       Master's or bachelor's degree from an accredited OT program

•       At least 1 year of clinical experience as an OT, home health experience strongly preferred

•       Current CPR certification

•       Reliable transportation and a valid Texas driver's license

•       Comfort with EMR documentation, WellSky experience preferred

•       Strong communication skills and professionalism with patients, families, and the agency care team
