Strive Health partners with skilled nursing and assisted living to deliver high-quality behavioral health care to residents. Our integrated model combines psychiatric nurse practitioners and therapists to provide a full spectrum of mental health services directly to residents int heir setting of care. We handle scheduling, credentialing, and billing so our clinicians can focus on what they do best: patient care.
Position Overview
We are seeking a Licensed Marriage and Family Therapist (LMFT) to provide psychotherapy and psychosocial support to residents in skilled nursing facilities. The LMFT will conduct assessments, deliver individual and group therapy, collaborate with the interdisciplinary team (IDT) and ensure regulatory compliance.
Key Responsibilities
  • Conduct initial diagnostic assessments and develop individualized treatment plans.
  • Provide psychotherapy services tailored to resident needs.
  • Deliver individual, group, and family counseling focused on depression, anxiety, grief, adjustment, and behavioral health issues.
  • Document sessions in compliance with Medicare and facility requirements.
  • Participate in care plan meetings and interdisciplinary team discussions.
  • Support residents and families in coping with illness, placement, and end-of-life issues.
  • Collaborate with Strive psychiatric nurse practitioners and facility staff to ensure coordinated care.
  • Educate facility staff on recognizing psychosocial needs and appropriate referral processes.

Qualifications
  • Hold an active LMFT license.
  • Master's in counseling or related field.
  • Enrolled or eligible for enrollment as a Medicare Part B provider.
  • Experience working with geriatric populations or in long-term care preferred.
  • Strong interpersonal, documentation, and communication skills.
  • Ability to work independently within a collaborative, team-based environment.
