Your Wellness Sources (YWS) is a mission-driven, integrated outpatient medical practice providing high-quality primary care, behavioral health, and wellness services to a culturally diverse population across the Maryland–DC metro area. Our care model emphasizes clinical excellence, continuity of care, collaboration, and compliance, with a strong focus on prevention and chronic disease management.
Position Overview
This is a part-time, onsite 1099 contractor position, working 2–3 consistent weekdays per week in our Hyattsville clinic. Remote or hybrid work is not available.
Core Responsibilities
  • Provide wellness visits, chronic disease management, and health assessments via secure telehealth platform
  • Order labs, prescribe medications, and provide patient education remotely
  • Review patient records, update care plans, and coordinate virtual follow-up
  • Accurately document all visits in AthenaOne EMR, ensuring compliance with billing and medical necessity standards
  • Collaborate with our in-house primary care, behavioral health, and nutrition teams for integrated patient care
  • Participate in provider team meetings, patient outreach, and care planning
Minimum Qualifications
  • Master’s Degree in Nursing and current Nurse Practitioner license (FNP or ANP) in Maryland
  • Active DEA and CDS license
  • ANCC or AANP certification
  • Minimum 3 year experience in outpatient primary care or internal medicine
  • Comfortable with telehealth platforms.
  • Proficiency with EMRs (eg. Epic, AthenaOne preferred)
  • Excellent communication, documentation, and clinical decision-making skillS
Preferred Qualifications
  • Experience working with high-risk, underserved, or medically complex populations
  • Familiarity with Maryland’s Prescription Drug Monitoring Program (CRISP)
  • Strong understanding of ICD-10, CPT coding, and evidence-based care documentation
