The
Medical Records Specialist is responsible for the timely and accurate import, indexing, scanning, and management of patient medical records within the Electronic Medical Record (EMR) system. This role ensures patient documentation is complete, properly categorized, and readily accessible to providers and staff to support quality patient care while maintaining strict confidentiality and compliance with HIPAA regulations.
Duties and Responsibilities
- Import electronic medical records from fax servers, referral portals, electronic interfaces, and secure email.
- Verify imported records are attached to the correct patient chart.
- Monitor electronic import queues and process documents in a timely manner.
- Identify and resolve import errors or duplicate documents.
- Ensure imported records are complete, legible, and available for clinical review.
- Accurately index imported and scanned documents into the appropriate patient chart and document category.
- Review patient identifiers to ensure proper chart placement.
- Classify documents according to established indexing standards.
- Correct indexing errors and reclassify documents when necessary.
- Merge duplicate records and documents as appropriate.
- Prioritize STAT and time-sensitive clinical documentation for immediate processing.
- Prepare paper records for scanning by removing staples, paper clips, and organizing documents.
- Scan paper documents into the EMR while maintaining image quality.
- Verify scanned images are complete, legible, and correctly oriented.
- Perform quality assurance checks on scanned documents before indexing.
- Maintain scanning equipment and report equipment issues when identified.
- Perform routine audits to ensure accuracy of imported, indexed, and scanned records.
- Maintain established quality and productivity standards.
- Identify discrepancies, missing documentation, or duplicate records and communicate concerns to the appropriate department.
- Continuously improve workflow efficiency while maintaining accuracy.
- Work closely with providers, clinical staff, scheduling, referrals, and other departments to ensure timely access to medical records.
- Respond professionally to requests regarding patient documentation.
- Assist team members during periods of high volume.
- Participate in cross-training and departmental process improvements.
- Protect all Protected Health Information (PHI) in accordance with HIPAA regulations.
- Maintain confidentiality of all patient records.
Follow organizational policies regarding document retention and destruction.
- Complete all required compliance and departmental training.
Qualifications
- High School Diploma or GED required.
- Medical office or Health Information Management education preferred.
- Previous experience in medical records, health information management, or a medical office preferred.
- Experience with Electronic Medical Record (EMR) systems preferred.
- Experience importing, indexing, or scanning medical records preferred.
Knowledge, Skills & Abilities

- Basic computer proficiency with experience using multiple software applications.
