**This position is based in Sebring, FL and requires daily travel within Highlands and/or Hardee County.  It is full-time, Monday thru Friday 8am - 5pm w/ participation in the on-call rotation.

Life as a Hospice RN Case Manager

Role of the RN Case Manager

As a Hospice Case Manager, you will have a special opportunity to provide, coordinate and direct the provision of hospice nursing and palliative care to terminally ill patients in their home and wherever they call home. Our Hospice nurses will closely collaborate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high-quality end-of-life care for the patient. We service an adult population providing support to patients and families.

Nuts & Bolts of the Job

Hospice RN Case Managers work Monday - Friday, 8AM - 5PM with a potential weekday on-call and Holiday coverage schedule. Case Manage 15-18 patients, attend Interdisciplinary Team Meetings, contribute to team huddle calls, and receive updates on your patients daily.

Provide Case Management:

  • Assign Aide Visits
  • Coordination of Care with Social Workers, Chaplains, Volunteers and Bereavement team
  • Develop and nurture relationships with facility staff and leaders throughout the community

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an R.N is 3-4 weeks, however, is individualized according to your needs and experience.

Responsibilities
  • Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
  • Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
  • Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
  • Records pain/symptom management changes/outcomes as appropriate.
  • Teaches patients, families, and caregivers appropriate patient care techniques.
  • Regularly communicates with clinical manager and care team.
  • Performs on-call responsibilities and on-call services to patients/families as assigned.
  • Other duties as assigned.

Qualifications

Required:

  • Current RN license in the state(s) where you will work
  • Minimum 1 year of clinical RN experience in an acute healthcare setting
  • Current CPR certification

Preferred:

  • Hospice or home health experience
  • Strong communication and interpersonal skills
  • Ability to work independently and manage multiple priorities
  • Comfortable with daily travel between facilities
  • Proficient in electronic medical record (EMR) systems
