Familiarity with electronic prescribing systems, drug databases, and medication management software is typically required. Strong attention to detail, communication skills, and patient counseling abilities are crucial for success. These skills ensure accurate medication assessments, enhance patient safety, and optimize therapeutic outcomes.

What is a medication review?

A medication review is a structured evaluation of a patient's medicines with the aim of optimizing the impact of drugs while minimizing related problems. It is typically performed by a pharmacist or healthcare provider and involves checking for drug interactions, ensuring the medications are still appropriate, and addressing any side effects or concerns. Medication reviews help ensure that patients are taking the right medicines in the correct doses and that they understand how and why to take them. These reviews are especially important for people who take multiple medications or have complex medical conditions.
