Position Overview:
Med-Care Providers is seeking a compassionate and experienced Nurse Practitioner (APRN-CNP) to provide in-home primary care services to homebound Medicare and Medicaid patients throughout Las Vegas and surrounding areas. This role is primarily focused on house visits, with the opportunity to also handle urgent care visits via telehealth as needed.
This is a flexible position ideal for providers seeking full-time or 2-3 fixed days per week with a consistent schedule.
Responsibilities:
  • Conduct comprehensive home visits for elderly and homebound patients
  • Evaluate and manage chronic and acute conditions
  • Perform medication reconciliation, lab reviews, and coordinate with interdisciplinary teams
  • Respond to urgent care needs via telehealth, as scheduled or on-demand
  • Complete accurate and timely documentation to support compliance and billing
  • Collaborate with care coordinators and specialists when needed
Schedule & Compensation:
  • Flexible: Full-time or minimum of 2-3 fixed days/week
  • Primarily in-person house visits, with telehealth urgent care as needed
  • Competitive per-visit or daily pay rate
Requirements:
  • Active APRN-CNP license in Nevada
  • Spanish Speaker
  • Must be Medicare and Nevada Medicaid credentialed (or eligible)
  • Minimum 1 year of experience in primary care, geriatrics, or urgent care
  • Reliable transportation for home visits
  • Tech-savvy and comfortable using telehealth platforms
  • Bilingual (English/Spanish) preferred but not required

The pay range for this role is:
80 - 90 USD per hour (Med-Care Providers)
