Job description

Community Care Givers is seeking a compassionate and skilled Registered Nurse (RN) to join our home health team as a Medicare Visit Nurse.

In this role, you will conduct admission assessments and skilled nursing visits, develop individualized care plans, and provide ongoing support to clients and their families. You will work closely with physicians, caregivers, and other healthcare professionals while ensuring compliance with Medicare guidelines and maintaining accurate documentation.


Qualifications:

  • Current RN license in the State of Michigan

  • Valid CPR BLS

  • Home health experience preferred

  • Knowledge of Medicare home health guidelines preferred
