About the Role

  • Community-based physician rounding in subacute skilled nursing facilities

  • Flexible scheduling - you control your day

  • Low census: 10-20 participants seen once weekly

  • Lead clinical oversight and quality for provider services

  • Collaborate with an experienced, supportive interdisciplinary team

  • Serve as a physician leader and mentor to NPs and other providers

  • Champion person-centered care for frail elderly participants
