As a Registered Nurse at Heritage Christian Services, you’ll work under the guidance of the Nursing Support Specialist to offer high-quality, person-centered care to people with intellectual and developmental disabilities. You’ll work alongside a dedicated nursing team to provide clinical oversight to frontline staff members, including direct support professionals and licensed practical nurses, and build meaningful relationships with the people you support and their families.

• Conducts ongoing health assessments and supports the health needs of people in certified residential, day habilitation and respite settings.
• Responds to health concerns and medical emergencies with clinical guidance andsupport.
• Partners with physicians, specialists,pharmaciesand medical liaisons to coordinate medical care and contribute to person-centered care planning
• Collaborateswith medicalliaisonstocoordinatetimelyschedulingofappointments, hospitalizations and related servicesandensuresresulting care plans and medical recommendationsare followed.
• Develops,implementsandmaintainsindividualizedPlans of Nursing Servicetocoordinatenursingcareanddefineresponsibilitiesfordirectsupportprofessionals.
• Delegates nursing tasks to direct supportprofessionalsinaccordance withthe New York State Nurse Practice Act and monitorsand evaluatestheir competency in health-relatedsupportsand delegated tasks.
• Oversees medication management processes, includingreview ofmedication administrationrecords andreconciliation.Addresses medication errors through investigation and follow-up.
• Participatesin incident management, including clinical review of reportable incidents.
• Ensuresall actions arecompliantwith OPWDD regulations, agencypoliciesand administrative memoranda.
• Supports agency surveypreparation, including participation in on-site surveysand remediation ofidentifieddeficiencies.

• Professionallicenseandcurrentregistration to practice as a registered nurse in New York stateasissuedby the New York State Education Department.
• Backgroundsupportingpeoplewithintellectual and developmentaldisabilitiesandpriorlong-term careexperienceare preferred.
• Mustdemonstratetheabilityandtemperamentnecessarytowork withand provide care to people withdisabilities.
• Validdriver’s license in compliance with Heritage Christian Services’ motor vehicle record standardsand reliable transportation to and from work.

Pay rate for this position: $73,500 / yr – $88,000 / year

The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above range represents the organization’s good faith and reasonable estimate of the range of possible compensation at the time of posting
