Health care helps.For nearly 35 years, Fourth Street Clinic has provided essential health care services to individuals experiencing homelessness in the Salt Lake City area. Joining our team means more than taking a job-you're directly supporting some of our community's most vulnerable members and helping them navigate their healthcare journey.Position OverviewThe Medical Case Manager (MCM) is a key member of the Medically Vulnerable Population (MVP) team, providing patient-centered case management for unhoused and medically vulnerable individuals. The MCM conducts needs assessments, develops personalized care plans, coordinates medical and behavioral health services, and facilitates access to supportive resources.
