CareGivers America, a Modivcare Personal Care Service, is looking for an RN/LPN responsible for providing high-quality services to our home care private duty clients.

  • PAY RATE $26-$30/hr

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide one-on-one, in-home care for a 17-year-old teen with complex medical needs related to a congenital condition affecting growth and development. This role focuses on promoting safety, comfort, independence, and quality of life in a supportive home environment.

Responsibilities Include:

  • Providing skilled nursing care in the home setting

  • Monitoring health status and responding to changes appropriately

  • Administering medications and treatments as prescribed

  • Assisting with activities of daily living as needed

  • Supporting mobility, positioning, and comfort

  • Communicating clearly with family members and healthcare providers

  • Maintaining accurate documentation and care records

  • Offering patient-centered, age-appropriate care with dignity and respect

Qualifications:

  • Current LPN or RN license in good standing

  • Experience with pediatric or adolescent patients preferred

  • Comfort caring for individuals with congenital or chronic medical conditions

  • Strong assessment, communication, and organizational skills

  • Compassionate, patient, and dependable
