Key Responsibilities:

  • Conduct thorough assessments of patient vitals, from blood pressure to heart rate, and document results meticulously for provider review.
  • Partner with healthcare providers to manage remote patient data and ensure timely follow-ups for any abnormal readings.
  • Energize communication between remote patients and healthcare providers, addressing inquiries with enthusiasm and guidance.
  • Keep our electronic health records (EHR) organized and accurate, ensuring all patient information is up-to-date and compliant with HIPAA regulations.
  • Collaborate with the healthcare team to craft and implement comprehensive patient care plans based on vital signs and clinical findings.
