What you will do: Conduct Comprehensive Assessments: Lead detailed interviews and evaluations using standardized state tools to determine clients' functional needs and eligibility for services. Assess formal and informal support networks and guide clients toward the appropriate care level.

Develop and Implement Service Plans: Create individualized care plans, authorize services, and coordinate implementation with clients, families, health professionals, and service agencies to ensure effective outcomes. Provide Resource Information and Referrals: Act as a knowledgeable resource for clients and caregivers, deliver community presentations, and refer to providers and interdisciplinary teams to ensure comprehensive care. Stay current on local and emerging services.

Maintain Client Documentation: Develop and maintain complete client files, documenting assessments, service authorizations, and follow-up actions. Ensure timely receipt of signed forms and applications to maintain program eligibility. Stay current on program and policy changes.

Perform Crisis Intervention: Provide supportive counseling and guidance to clients and caregivers who are vulnerable, functionally impaired, or unable to care for themselves. Represent the Division Professionally: Serve as an ambassador for the department, providing outreach, education, and marketing services to the public as needed. What you bring: Educational/Professional Experience Bachelor's degree from a college or university accredited by an agency recognized by the U.S

Department of Education in social work, Human Services, Psychology, or a related field AND two years of paid, on-the-job social service experience. OR A general bachelor's degree from a college or university accredited by an agency recognized by the U.S. Department of Education AND four years of paid, on-the-job social service experience

Technical/Industry Background: Proficiency with state and federal program regulations affecting adults with functional impairments. Skilled in using assessment tools, care planning software, and maintaining accurate client documentation. Specialized Expertise: Knowledge of local social services, healthcare systems, and community resources.

Expertise in crisis intervention, discharge planning, and caregiver support strategies. Communication/Interpersonal Skills: Strong interpersonal skills to engage with clients, families, and community partners. Excellent written and verbal communication for assessments, reports, and community presentations.

Additional Preferred Qualifications: Masters Degree in behavioral or health science Experience working with diverse populations and vulnerable adults. This recruitment is being used to fill an existing open position and may be used to fill future openings across all Aging divisions for up to six (6) months. Work hours for this position are typically Monday - Friday, 8 a.m. - 4:30 p.m

Driving Requirements The successful incumbent must meet the driving requirements of this position and comply with the Kitsap County Vehicle Use Policy. The successful incumbent must obtain a valid Washington State Driver's License prior to hire and maintain a satisfactory driving record as outlined in the Kitsap County Vehicle Use Policy. This position is categorized as: Category 2: Driver operates a personal vehicle as an essential function for official County business.

Any employee using a personal vehicle on official County business shall be required to maintain auto liability insurance, in addition to having a valid driver's license. Criminal Conviction Standards: The successful incumbent will or may have unsupervised access to children under 18 years of age, a vulnerable adult, or a vulnerable person, or access to a secured/confidential facility or computer system(s).
