MDS Coordinator Nursing Home Facility Full-time Day Shift Mon-Fri

MDS Coordinator RN Position Overview:

Harmony Care Group is now hiring a full-time MDS Coordinator. RN is responsible for implementing the federally mandated assessment tool to identify the concerns of the residents and to coordinate a plan of care to assist the residents toward optimal wellness based on the highest practicable level of functioning.

MDS Coordinator RN Key Duties:

  • Manage the overall process and tracking of all Medicare/Medicaid case-mix documents to assure appropriate reimbursement for services provided within the nursing center.

  • Conduct concurrent MDS reviews to assure achievement to maximum allowable RUG categories.

  • Integrate information from nursing, dietary, social services, restorative, rehabilitation, and physician services to ensure appropriate reimbursement.

· Perform other job-related duties as they become required.

MDS Coordinator RN Qualifications:

  • MDS/RAI experience. Skilled Nursing Experience Preferred.

  • Must have Registered Nurse (RN) certification in PA.

  • Knowledgeable of all Medicaid legal regulations, guidelines, and best practices.

  • Demonstrated skills in Microsoft Word and Excel. Power Point and Outlook, Internet navigation.
