Job Title: Registered Nurse - Hospice
Location: Rutland, VT
Type: Contract (13 Weeks)
Shift & Schedule: Tuesday - Saturday | 8:00 AM - 4:00 PM | Some On-Call Coverage Required | Holiday Coverage Required
Pay: $50/hr - $63/hr

Position Overview:

Navitas Healthcare, LLC is seeking a Registered Nurse - Hospice for an exciting Local or Travel job in Rutland, VT.

Responsibilities

  • Perform comprehensive patient and caregiver assessments.

  • Develop, implement, and evaluate individualized plans of care.

  • Serve as Case Manager for assigned hospice patients.

  • Participate in Interdisciplinary Team (IDT) meetings and care planning.

  • Coordinate patient care with physicians, vendors, caregivers, and healthcare providers.

  • Provide pain management, symptom management, wound care, IV therapy, and other skilled nursing interventions.

  • Educate patients and families regarding medications, treatments, disease progression, and available services.

  • Maintain accurate and timely documentation in McKesson EMR.

  • Supervise Hospice Aides and LPNs according to regulatory requirements.

  • Conduct supervisory visits and ensure compliance with state and federal hospice regulations.

  • Facilitate admissions, transfers, discharges, and care transitions.

  • Participate in quality improvement activities, peer reviews, and team meetings.

  • Advocate for patient and family needs to ensure high-quality, compassionate care.

Required Qualifications

  • Active Registered Nurse (RN) License (Vermont license obtainable upon acceptance).

  • Minimum 2 years of Hospice RN experience.

  • Strong assessment, case management, and care coordination skills.

  • Experience with end-of-life care and symptom management.

  • Ability to work independently in home and community settings.

  • Excellent communication and documentation skills.

Preferred Qualifications

  • Experience using McKesson EMR.
