Registered Nurse (Home Health)

Location: Rutland, Vermont

Pay Rate: $2500 To $2720/Week

Contract: 13 Weeks

Schedule: Monday–Friday, 8:00 AM – 4:00 PM

Experience: Minimum 2 years of Home Health nursing experience

About the Role

We are seeking an experienced Home Health Registered Nurse (RN) to provide compassionate, patient-centered care in the home setting. The RN will perform comprehensive patient assessments, develop individualized care plans, manage caseloads, and coordinate care with interdisciplinary teams while ensuring compliance with agency, state, and federal regulations. OASIS experience is required.

Responsibilities

  • Perform comprehensive patient assessments and develop individualized care plans using the nursing process.

  • Complete accurate OASIS documentation and maintain timely electronic documentation in McKesson EMR.

  • Manage assigned patient caseloads, coordinate care with physicians and interdisciplinary teams, and serve as the primary case manager.

  • Provide skilled nursing care, including IV therapy, wound care, wound VAC management, medication administration, and patient/caregiver education.

  • Monitor patient progress, coordinate safe discharges, maintain infection control standards, and ensure compliance with agency, state, and federal regulations.

Requirements

  • Current Registered Nurse (RN) license (Vermont license upon acceptance).

  • Minimum 2 years of Home Health nursing experience.

  • OASIS experience is required.

  • Experience with McKesson EMR preferred.

  • Strong case management, documentation, communication, and patient education skills.
