Job Summary

Provide direct nursing care in a skilled nursing facility supporting long-term care, rehabilitation, short-stay, and dementia residents. Responsibilities include medication administration, patient monitoring, documentation in MatrixCare EMR, and collaboration with the interdisciplinary care team to ensure safe, high-quality resident outcomes.


Requirements
  • Active LPN license (WI or Compact)

  • Experience in Skilled Nursing Facility (LTC preferred)

  • Ability to work evening shift (2p–10:30p)

  • Experience with MatrixCare EMR preferred

  • Strong medication administration and documentation skills

  • Ability to work independently and within a team

  • Strong communication and patient care skills

  • 43-bed Skilled Nursing Facility

  • Census: 37–38 residents

  • 2-unit layout (2 halls)

  • Mixed population: LTC, Rehab, Short Stay, Dementia Care

  • MatrixCare charting system

  • Any color scrubs allowed

  • Some 12-hour shifts available

  • Rural facility (no public transportation)

  • Unique environment with animals in lobby (birds/fish)
