Overview:
The Lead MOUD Registered Nurse provides clinical and programmatic leadership for the Medication for Opioid Use Disorder (MOUD) program at the Fallsway site of Health Care for the Homeless, a low-barrier, walk-in program serving individuals with substance use disorders and complex health and social needs. Grounded in trauma-informed care, harm reduction, and client-centered philosophies, this role supports a full continuum of engagement—from harm reduction and safer-use services to treatment initiation, ongoing engagement, and connection to primary care, behavioral health, and other services. The Lead MOUD RN is responsible for supervising MOUD nursing staff, supporting nursing practice standards, coordinating clinic workflows, and leading continuous quality improvement efforts. Working in close partnership with the Lead MOUD Medical Provider, Director of Recovery Services, Senior Medical Director–Fallsway, operations colleagues, and interdisciplinary team members across the organization, this role helps ensure accessible, coordinated, whole-person care for clients accessing the MOUD clinic.

Key Role Responsibilities:
Clinical Care and Practice Leadership:
  • Provides direct nursing care within the MOUD program, including addiction care, wound care, harm reduction services, screening, care coordination, and urgent, chronic, and preventative nursing care when appropriate.
  • Maintains an active clinical practice and models person-centered, trauma-informed, and harm reduction approaches to care.
  • Provides clinical coaching, onboarding, mentorship, and supervision to MOUD nursing staff and other team members supporting the clinic.
  • Directly supervises assigned staff and provides clinical oversight, guidance, and support for all nursing staff delivering care within the MOUD clinic.
  • Supports integration of preventive care, chronic disease management, and health screenings into MOUD services.
  • Completes documentation in accordance with agency standards and required timeframes.

Clinic Coordination and Program Implementation:
  • Coordinates day-to-day clinic functions including staffing, scheduling, workflow development, clinic communications, and supply management.
  • Supports implementation and continuous refinement of clinic processes that promote efficient, responsive, and low-barrier access to care.
  • Collaborates with the Director of Recovery Services and other partners to ensure the availability of harm-reduction and safer-use supplies.
  • Facilitates effective communication and collaboration among MOUD team members and interdisciplinary partners.

Quality Improvement, Compliance, and Environment of Care:
  • Leads clinic-level quality improvement efforts to improve access, client experience, and clinical outcomes.
  • Oversees nursing quality, environment of care, and clinic compliance activities, including point-of-care testing, medication storage, staff competencies, and response to audit findings.
  • Maintains a safe, clean, and appropriately stocked clinical environment.
  • Supports compliance with agency policies, accreditation standards, regulatory requirements, and nursing practice standards.

Collaboration and Care Integration:
  • Cultivates collaborative relationships with behavioral health, supportive services, outreach, operations, primary care, and community partners.
  • Supports warm handoffs, care coordination, and interdisciplinary care planning to advance whole-person care.
  • Collaborates with clinical and organizational leaders to advance program goals and improve client outcomes.
  • Supports relationships with external treatment programs and community partners that enhance access to addiction treatment and recovery support services.

Key Agency Responsibilities: In addition to role responsibilities, each staff member of Health Care for the Homeless has the following responsibilities as a part of their employment:
