Full-Time Social Services Director

Address: 337 Neponset Ave Dorchester

The Bostonian Nursing and Rehabilitation is seeking a Director of Social Services with up to a $7,500 Sign-On Bonus, to join and support our fantastic team caring for our short-term patients and long-term residents.  Accessible by public transportation and with a private parking lot, this this facility is located in the heart of Dorchester. 

Requirements:

  • Must possess a minimum of a B.A in Social Work, LSW or equivalent degree AND a current social work license.

  • Masters in social work (MSW), LCSW, or LICSW highly preferred.

  • 1+ year(s) skilled nursing facility experience, preferred

Job Duties:

  • Compiles social history data.

  • Participates in assessments, development and evolution of resident's care plan.

  • Responsible for planning and leadership interdisciplinary care planning weekly meetings.

  • Documents timely progress notes according to state and federal regulations.

  • Coordinates the discharge planning for residents.

  • Able to interpret and implement established social service policies, procedures and regulations.

  • Coordinates services of community agencies for continued delivery of care upon discharge.
