Social Work/Care Companion

Join Carease Health - PA in Hillsdale, PA

As a compassionate and dedicated Mental Health Provider, you'll provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. At Carease Health, we prioritize proactive measures for enhanced physical health and well-being, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our comprehensive preventative care program.

Key Responsibilities:

  • Visit designated facilities in accordance with the schedule set with your supervisor

  • Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs

  • Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns

  • Document visit notes and maintain detailed records of interactions and interventions after each visit

  • Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications:

  • Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field

  • Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings

  • Strong communication, documentation, and relationship-building skills

  • Education: Associate Degree

  • Experience: 1 year
