• Conduct thorough assessments of the mental health needs of clients, creating tailored treatment plans that include psychotherapy, crisis intervention, and support services.

• Work as an integral part of a multidisciplinary primary care team, effectively communicating patient needs and collaborating on treatment approaches with physicians and other healthcare providers.
• Manage and maintain a diverse caseload of clients, ensuring timely follow-up and adherence to established mental health policies and procedures.
• Evaluate and assess new patients through a courteous intake process while ensuring a warm and welcoming atmosphere.
• Provide immediate crisis intervention as required, offering support and guidance to individuals in distress.
• Refer clients with severe mental health conditions to appropriate inpatient or specialty care services as necessary.
• Educate patients on healthcare resources available within the community, including medication-assisted treatment (MAT) services for substance use disorders.
• Screen clients for substance abuse and provide appropriate counseling, or refer them to local support resources as needed.

• Master’s degree in Counseling or Social Work from an accredited institution.
• Current and valid LCSW license to practice in Nevada.

• Strong assessment skills with a solid understanding of mental health disorders commonly faced by hospice patients.
• Excellent oral and written communication skills, demonstrating ability to effectively present and share information with a multidisciplinary team.
• Proficiency in digital communication tools and electronic medical record systems to document patient interactions and provide seamless care.
