We are hiring a Medical Social Worker (MSW) for a Hospice position providing care to patients in their homes in the Lake Orion area.

What You’ll Do:

  • Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
  • Conduct comprehensive psychosocial and bereavement assessments, identifying patient and family needs while developing care plans that support emotional, social, and end-of-life goals
  • Provide counseling, emotional support, and guidance to patients and families, connecting them with resources and helping them navigate the end-of-life journey with clarity and dignity

MSW Qualifications

  • Master’s in Social Work from graduate school accredited by the Council of Social Work
  • One (1) to two (2) years post graduate social work experience in a health care setting
  • Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work
  • Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
