Mountain Community Health is a Federally Qualified Health Center and recognized Patient-Centered Medical Home serving roughly 4,300 patients across the five-town Bristol area with primary care, dental, and behavioral health services. We care for everyone who walks through the door, regardless of ability to pay.
Quality is not a reporting function here. UDS, PCMH recognition, and our grant commitments all depend on this work being right. You will own the technical integrity of the data and the improvement work it drives.
What you will own
โ€ข Quality measure definitions, data sources, and reporting logic, documented so results are reproducible.
โ€ข Standardizing our eClinicalWorks reporting so the same question returns the same answer every time, and leading UDS reporting end to end.
โ€ข Dashboards and reports for clinical teams, leadership, the Quality Committee, and the Board.
โ€ข Day to day QI/QA program: quality calendar, PDSA cycles, corrective actions, HRSA and PCMH documentation.
โ€ข Registries, care gap reports, and stratified analyses that surface disparities in access and outcomes.
Who you will work with
Quality here is co-led. You will partner with our Quality Clinical Lead, a practicing physician who holds protected hours every week for this work. You own the data, the systems, and the program mechanics. They own clinical interpretation and provider engagement. You will also work closely with our Medical Director, Nurse Manager, and Director of Operations. You will have a physician partner, an executive sponsor, and a nursing team that acts on what you find.
Requirements
What we are looking for
  • Demonstrated ability to pull, clean, validate, and analyze healthcare data independently, including advanced Excel and EHR-based reporting.
  • A track record of turning data into something a non-technical audience acted on.
  • Strong project management and follow-through. This role owns deadlines that do not move.
  • Comfort telling a physician or an executive that the data does not support what they hoped it would.
  • Bachelor's degree in public health, health informatics, data analytics, or a related field. Equivalent experience considered.
  • Preferred: FQHC or PCMH experience, HRSA and UDS reporting knowledge, eClinicalWorks reporting including eBO, SQL or Power BI or Tableau, and PDSA or Lean methodology.

Our EHR is eClinicalWorks. If you have used it, you know reporting takes patience. We will fund the tooling you need and back you when a measure cannot be pulled cleanly.
