Job description

Our Mission and Philosophy:
Personal choice, community involvement, and dignity while thriving in a caring and respectful atmosphere.  These four principles make up the hallmark philosophy.  Based on the above four principles; our Supplemental Support Program is designed to assist adults with intellectual disabilities live and thrive in the community, achieve their goal and sustain good health. The Supplemental Support Program will provide assistance to adults with moderate to severe deficits in self-help skills; and/or with moderate to severe behavioral challenges. Our program is also geared to serve adults who may have a mental illness and in need of proper support to live successfully in the community. The program will enable individuals to overcome barriers at home or out in the community and be integrated into everyday living situations, such as attending school, educational programs, public services, after-school programs, and recreational activities.
SUMMARY
The SLS Lead is responsible to assist the Case Manager/SLS Supervisor with the coordination of the individuals so that the residents’ health and safety needs are met. The SLS Lead assists to maintain compliance under the California Code of Regulations (CCR), Title 17.The SLS Lead is expected to ensure residents to maximize independence.
Knowledge
Knowledge of regional centers, department of social services, department of developmental services, and state council on developmental disabilities. Title 17 and dual diagnosis, and IPP.
Skills And Abilities 
Verbal communication skills, reading and writing comprehension, time management, clinical skills, interpersonal, flexible, behavioral management, and problem solving. Ability to engage in service recipients’ hygiene, manages behavior problems, and meets other challenges. Ability to handle sensitive issues while protecting others' welfare, ability to be honest, reliable, dependable, and professional at all times. Ability to be organized and task oriented.
EDUCATION and EXPERIENCE

Education
High school diploma or GED. CPR and First Aid.
Experience 
One-Three year experience with direct supervision and services to persons with developmental disabilities.
ESSENTIAL FUNCTIONS

  • Provide oversight of all household activities assuring timely and high quality completion of all tasks
  • Work with employees, consumers, and outside vendors assuring the best
  • quality of work, and adherence to all household standards and expectations
  • Follow physician's directions, including assisting the client with the administration of medication and other treatment
  • Implement approved emergency procedures in the event of a fire, earthquake, or other natural disasters
  • Participate in the development of the Individual Program Plan (IPP)
  • Provide transportation to medical and dental appointments, community outings and as otherwise outlined in the IPP - using company approved vehicle (this may include company vehicles and employee's vehicle)
  • Maintain a record of medical and dental visits, current weight, medication (dosage), frequency and times of administration, reasons for medications, (side effects), illnesses, injuries, special incidents, funds, and personal possessions etc.
  • Actively manage and monitor each individual’s financial obligations, including rent and utilities to ensure payments are completed on time. Includes verifying payments status, maintaining communication with payees or service providers and immediately addressing any past due or at risk accounts. Employee(s) are required to document and related activities and notify supervisor or any missed or delayed payments within 24 hours.
  • Develop a shopping list to purchase food and supplies as instructed by the
  • Administrator to prepare meals according to individuals desire and encourage individuals to assist and learn.
  • Communicate on a regular basis with Regional Center, day programs, and other services providers and agencies, as needed
  • Maintain a master calendar of appointments, facility activities, meetings with Regional Center, etc.
  • Support SLS Specialist to maximize the clients’ independence
  • Perform other related duties and assignments are required
PHYSICAL REQUIREMENTS
The SLS Lead position will typically work in the homes and occasional work in  an office environment and in small enclosed spaces. Frequent sitting, fingering, grasping, feeling, talking, hearing and repetitive motions. Occasional lifting of 50 lbs. Occasional bending, twisting, stooping, squatting, pushing, pulling, standing, crawling, reaching, walking, driving, traveling, kneeling and crouching.
OTHER REQUIREMENTS

  • Must be 18 years or older
  • Must possess a valid California Driver’s License and automobile insurance
  • Must have a minimum of one to three year experience supporting adults with developmental disabilities with Mental Health
  • Must possess a valid CPR/FA certificate or obtain one within one week of beginning employment
  • Required to pass a background check
