Position Summary
The SLS Coordinator supports the day-to-day coordination of Supported Living Services for adults with intellectual and developmental disabilities. This position helps coordinate staffing, schedules, service documentation, authorizations, ISP meetings, participant support needs, and communication among participants, families, support staff, Regional Center representatives, and HCAR leadership.
The SLS Coordinator works in both administrative and community-based settings. The position is expected to be approximately 70% office-based and 30% out-of-office, including participant homes, community settings, meetings, appointments, and service locations. The position may also provide coverage for vacations, staffing gaps, or participant service needs as assigned.
This role supports person-centered services, health and safety, service continuity, accurate documentation, billing support, and participant independence while following HCAR policies, funding requirements, confidentiality standards, and applicable regulatory expectations.
Essential Duties and Responsibilities
Essential duties and responsibilities include the following. Other duties may be assigned based on program and organizational needs.
• Help coordinate daily SLS services to ensure participants receive appropriate support, staffing, and follow through based on individual needs, schedules, and service plans.
• Coordinate support staffing coverage as assigned, including planned absences, vacation coverage, open shifts, and short-term coverage needs.
• Maintain accurate participant schedules, service documentation, authorization-related records. Monitor participant service hours, authorized services, and staffing needs to support accurate billing, service delivery, and compliance with funding requirements.
• Follow up on participant authorizations, service approvals, POS information, and related communication with Regional Center representatives or other funding sources as assigned.
• Schedule ISP meetings and coordinate with participants, families, service coordinators, support staff, and other members of the participant support circle.
• Send ISP documents, reports, meeting materials, or other participant information as requested and in accordance with confidentiality requirements.
• Maintain accurate participant files, contact information, emergency information, case notes, reports, schedules, authorizations, and other service-related documentation.
• Support participants in developing personal goals, decision-making skills, independent living skills, community access, and use of natural or generic resources.
• Coordinate with support staff and the participant support circle to identify needs, address barriers, solve problems, and maintain consistent service delivery.
• Provide support and training in home, community, transportation, mobility, social, recreational, and daily living environments as assigned.
• Assist participants with access to benefits, appointments, community resources, activities, and services necessary to support health, safety, and independence.
• Identify home or community health and safety concerns and assist with emergency planning, crisis response documentation, and follow-up communication as assigned.
• Use Microsoft Suite, scheduling systems, EVV technology, email, spreadsheets, and other documentation tools to support program operations and service tracking.
• Prepare routine correspondence, reports, spreadsheets, schedules, case notes, billing support, and other administrative documents as needed.
• Provide direct service coverage when needed for vacations, staff absences, open shifts, participant appointments, or other program needs as assigned.
• Maintain professional communication with participants, families, support staff, service coordinators, community partners, and HCAR leadership.
• Follow HCAR policies, confidentiality requirements, participant rights standards, mandated reporting expectations, safety procedures, and applicable program requirements.
• Perform other duties as assigned to support participant services and program operations.
Supervisory Responsibilities
This position does not provide formal supervision unless specifically assigned. The SLS Coordinator may provide scheduling coordination, daily direction, training support, and guidance to support staff assigned to participant services. The position is expected to communicate staffing, performance, safety, documentation, or service concerns to the Program Manager, Program Director, or assigned supervisor.

Minimum Requirements
• High school diploma or GED required; related college coursework or education in human services, social services, psychology, rehabilitation, special education, or a related field preferred.
• Minimum of two years of experience working with individuals with intellectual or developmental disabilities, supported living services, independent living services, direct support, case coordination, or a comparable setting preferred.
• Knowledge of person-centered planning, individual-driven services, community integration, supported living principles, participant rights, and confidentiality expectations.
• Ability to help coordinate schedules, support staffing, service coverage, vacation coverage, participant appointments, and ISP-related meetings.
• Ability to follow up on participant authorizations, service approvals, documentation needs, and communication with Regional Center representatives or other service partners.
• Knowledge of Microsoft Suite, including Word, Excel, Outlook, and related office systems.
• Ability to learn and use EVV technology, scheduling systems, documentation platforms, email, spreadsheets, and other program tools.
• Ability to communicate clearly and professionally with participants, families, staff, service coordinators, community partners, and leadership.
• Ability to maintain organized records, complete accurate documentation, prepare reports, and manage follow-up tasks within required timelines.
• Ability to work independently, exercise sound judgment, prioritize competing tasks, and seek supervisory guidance when needed.
• Valid California driver license, reliable transportation, proof of automobile insurance, and acceptable driving record required when driving is part of assigned duties.
• Ability to successfully complete required background clearance, fingerprinting, TB clearance, physical examination, and other employment-related screenings.
• Completion and maintenance of required agency training, including mandated reporter training, confidentiality training, participant rights training, emergency procedures, and other required training within established timelines.
Physical Demands
The physical demands described here are representative of those required to perform the essential functions of this position. The employee may regularly sit, stand, walk, talk, hear, use hands and fingers, reach, bend, and use a computer, tablet, phone, mobile device, or other office and program equipment. The position requires travel to participant homes, community settings, appointments, meetings, and assigned service locations. The employee must occasionally lift and move up to 25 pounds.
