Are you a compassionate nursing leader who is passionate about improving the lives of older adults? HopeWest is seeking an experienced Clinic Manager (RN) to lead the daily operations of our innovative PACE clinic, where you'll guide an exceptional interdisciplinary team and help ensure participants receive coordinated, high-quality care that allows them to remain safely and independently in their homes.

As the Clinic Manager, you'll oversee the day-to-day clinical operations of our PACE center, ensuring exceptional participant care, efficient workflows, and regulatory compliance. You'll mentor and support clinical staff, foster a collaborative team culture, and partner with the interdisciplinary team to deliver person-centered care that reflects HopeWest's commitment to excellence.

What is PACE?

PACE (Program of All-Inclusive Care for the Elderly) is a unique Medicare and Medicaid program designed to help older adults remain living safely and independently in their own homes for as long as possible.

Through an interdisciplinary approach, PACE provides comprehensive medical care, rehabilitation, transportation, social services, medications, and other essential supports—all coordinated through one dedicated team.

With more than 200 PACE programs serving over 95,000 participants across 33 states, the model continues to grow as communities recognize the value of coordinated, whole-person care.

What You'll Bring

Required Qualifications

  • Bachelor's degree in Nursing (BSN)

  • Current, unrestricted Colorado Registered Nurse (RN) license

  • Leadership experience in a healthcare setting

  • Valid Colorado driver's license, reliable transportation, and required automobile insurance

  • Current CPR certification

Preferred Qualifications

  • Three (3) or more years of clinic management experience
