Here’s What You’ll Do for Our Residents & Facility:

Care Needs & Planning: Assist to develop, implement and continually update individual care plans, supplement hands on care needs

Assessments: Complete admission, discharge and other assessments as needed

Triage & Interventions: Assist to handle acute health changes, physicians' orders and collaboration with care providers & families

Regulatory Standards: Ensure compliance with federal, state, county & company health/safety regulations

Quality Assurance: Audit documentation & care for quality control/compliance

Family & Team Liaison: Communicates changes in resident conditions to families/providers, works collaboratively with the interdisciplinary team

Team Oversight & Mentorship: Provide ongoing training, orientation & performance evaluations for staff. Manage staffing schedules and assignments
