BEHAVIORAL HEALTH CLINICIAN

Professional

Full Time Exempt


The Behavioral Health Clinician performs mental health and/or substance use disorder outpatient counseling. The position works from an integrated behavioral health model providing a broad spectrum of culturally appropriate services, including completing behavioral health assessments with standardized behavioral and health risk screenings, providing group and individual counseling, help with case management, and make referrals to students with behavioral health/academic needs. All of these duties are required to be done in accordance with the policies and procedures set forth by Camillus Health Concern.


Key Responsibilities

  • Provide behavioral health outpatient counseling (Mental Health, Substance Use Disorder, Co-Occurring) to patients
  • Assist with screening, identification and management of patients with behavioral health difficulties, victims of abuse, neglect, domestic violence and trauma.
  • Provide assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, academic progress, and/or life situation.
  • Obtain and study medical, psychological, social, and family histories, by interviewing patients and by reviewing records.
  • Provide psycho education to patients around issues related to adaptation to the patient’s illness.
  • Develop and implement treatment plans, specifying type, frequency, intensity, and duration of therapy.
  • Evaluate the effectiveness of treatments, and the accuracy and completeness of diagnoses, and then modify plans and diagnoses as necessary.
  • Consult / collaborate / coordinate with the BHS Team in the development and implementation of treatment plans whenever required or clinically relevant.
  • Monitor clients in person or by telephone for changes in clinical symptoms and treatment side effects or complications.
  • Perform case management and provide social services when appropriate.
  • Provide brief interventions using evidence-based techniques such as behavioral activation, motivational interviewing, or other relevant skills.
  • Provide or facilitate in clinic or outside referrals to evidence-based psychosocial treatments such as the Cognitive Behavioral Therapy, Problem Solving Treatment, Stress Management Training, Dialectical Behavioral Therapy, and Behavioral Activation as needed.
  • Conduct regularly scheduled caseload consultation with BHS Team.
  • Facilitate patient engagement and follow-up in health care.
  • Track patient treatment, follow-up, and clinical outcomes in the medical record and/or other databases as required.
  • Document in person and telephone encounters in medical record and other databases. Use database and medical record system to identify and reengage patients who may be lost to follow-up.
  • Document services rendered in patient electronic records and/or other databases as required.
  • Facilitate treatment plan changes for patients who are not improving as expected in consultation with the PCP and other behavioral health providers. These may include referrals for additional services.
  • Facilitate referrals for clinically indicated services outside behavioral health (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, in-patient substance abuse treatment).
  • Facilitate program workflow and improvements at his/her site so as to achieve program quality and performance aims.
  • Participate and represent behavioral health perspective in multidisciplinary meetings and trainings.
  • Support reporting requirements per grant contracting requirements.
  • Maintain professional and technical knowledge by attending educational workshops and reviewing professional publications.

Education & Experience

  • Licensed Clinical Social Worker (LCSW)/ Mental Health Counselor (LMHC) for FL
  • 2-3 year experience providing therapy.
  • Bilingual (English/Spanish) preferred, Creole a plus
  • Ability to work in a highly diversified environment.
  • Maintains active valid licensure and credentials.
  • Excellent organizational and documentation skills
