Director of Case Management
Puyallup, Washington

The RN Director of Case Management leads a dynamic team of clinical professionals in delivering exemplary cross-continuum Care Coordination and Care Transitions. The Director develops and implements the Case Management process, supervises clinical staff, promotes high-quality patient outcomes, and supports appropriate Utilization and Resource Management. The Director identifies improvement opportunities, executes collaborative practices that exemplify patient-centered care and community engagement, and works closely with market leaders, physicians, peers, community colleagues, and system leaders.

Qualifications:

  • Registered Nurse (RN) with acute hospital nursing experience required
  • Bachelor Degree required
  • Director of Case Management in an Acute Hospital Setting required
  • Bachelor's or Master's in Nursing (BSN/MSN) preferred
  • Certification in Case Management (CCM/ACM) preferred

Responsibilities:

  • Coordinates and supervises the duties of the clinical Case Managers and Social Workers
  • Promotes quality patient care outcomes while supporting appropriate utilization/resources management
  • Facilitates the multidisciplinary Case Management process by coordinating the development and implementation of tools and systems for effective, efficient Case Management
  • Identifies opportunities to continually improve patient care and services
