Job description

Summary of Functions
To guide, educate, and support individuals through positive goal-oriented interventions to strengthen and empower them towards self-sufficiency. Case manager will access client's needs and then connect them with the necessary community resources by guiding them through the social support network systems. This position reports to the Behavioral Health Administrator and office is based on the KTI reservation in Usk, WA.
Essential Duties and Responsibilities
  • To provide wrap around services for individuals referred from agencies and programs affiliated with the Kalispel Tribe of Indians.
  • The case manager will work collaboratively with Camas Path Behavioral Health, community service programs and agencies to ensure the coordination of treatment services.
  • Maintain client and program records in accordance with Tribal, State and Federal laws.
  • As a team member, the case manager will work collaboratively with staff, colleagues, the Tribal and local court systems, and all other agencies that provide services to the Kalispel Community, as well as the community at large.
  • Must be able to transport clients and attend appointments, meetings, court hearings and other appointments in the community or out of state if necessary.
  • Case manager will provide outreach, intervention, evaluations, treatment planning, case management, aftercare services, advocacy and referrals as needed.
  • Case Manager will provide clients with relevant and easy to comprehend information about tribal and community services.
  • Adhere to all applicable tribal, federal, or state privacy laws and regulations, including but not limited HIPAA and 42 CFR Part 2.
  • This position supervises and oversees the Camas Path Transporter and the south Case Manager.
  • Attend training as necessary.
  • Additional duties as assigned

Education
  • Minimum of BA or BS Degree in Social Work or related Social Services field OR an Agency Affiliated Certificate from the Department of Health and two years work experience with vulnerable populations is required.
  • If has a BA or BS Degree in Social Work or related Social Services field, must meet the qualifications at hire to obtain an Agency Affiliated Certificate from the Department of Health is required.
  • Must obtain an Agency Affiliated Certificate from the Dept of Health within 90 days of hire.

Experience
  • Requires 3 years of experience working with vulnerable populations.
  • Requires 1 year experience working with Native American People and Tribal Organizations.

Other Requirements
  • Must have a valid driver's license, reliable transportation and be eligible for automobile insurance.
  • Must have or be able to obtain HIV Aids Training and a current CPR and First Aid Certification.
  • Must be able to pass a background check and sign a statement of confidentiality.
  • This position requires some amounts of work to be performed outside general business hours.

Physical Demands
  • This position requires the ability to bend, stoop, lift, carry, and move items weighing up to 20 pounds.
  • This position requires significant amounts of standing or walking.
  • Significant portions of the job require extended sedentary periods; constant work with computers, including extended periods of keyboarding; and repetitive motion of hands and wrists.

Work Environment
  • This position is performed primarily in an office environment.
  • This position requires local and out of state travel.
  • This position requires the ability to safely operate a motor vehicle.
  • On-call status may be necessary at times.
